Often, no. I've told plenty of owners their site was fine and the problem was three paragraphs of unclear copy. That's a few hundred dollars of work, not a rebuild.
A full redesign earns its cost when the structure itself works against the visitor — when the site is slow, unusable on a phone, impossible to update, or built around what you wanted to say rather than what a buyer needs to decide.
